Coordinator, SNF Nursing Care

JOB PURPOSE: Administer, manage, interpret and authorize coverage of the skilled nursing facility care benefit. Monitor and evaluate the quality of care provided by contract SNFs. Work in partnership with Kaiser Permanente clinicians, facility staff and administrators to reduce adverse outcomes and enhance patient safety and satisfaction.
EDUCATION REQUIREMENT: High school diploma or GED. Bachelors of nursing or equivalent knowledge/experience.
LICENSING/CERTIFICATION REQUIREMENTS: Requires Oregon or Washington license at time of hire. Licenses in both states are required within six (6) months of hire. Current, valid drivers license.
MINIMUM QUALIFICATIONS: Two or more years of medical/surgical or home health/hospice nursing experience. Working knowledge of utilization/quality management. Working knowledge of Medicare benefits and their application in the long term care or home health/hospice arena. Demonstrates customer-focused service skills. Well-developed patient assessment and care planning skills. Well-developed and effective verbal and written communication skills.
Demonstrates warmth, cultural competence and responsiveness to customer needs.
PREFERRED QUALIFICATIONS: Experience with monitoring and management of patients in nursing homes. Thorough knowledge and experience with skilled nursing facility operations and regulations. Working knowledge of Health Plan benefits and their application. Thorough knowledge and experience with interpreting Medicare regulations and guidelines.
HOURS/SHIFTS: 40hrs per week. Days, Monday thru Friday, 8:00am to 5:00pm. This position is located at the Montgomery Park Office.
