Disease Management Nurse, R.N.: Kern Health Systems

The Disease Management Nurse is responsible for the coordination, collaboration, education and management of members identified to be in high risk categories. This person will work under the Director of Quality Improvement, Health Education and Disease Management. Qualifications are: Valid California nursing license, Ten (10) years of experience as a registered nurse, at least five (5) years in an acute care hospital on a medical/surgical ward or unit or in the medical/surgical intensive care unit preferred. Patient education and previous utilization management experience in a managed care setting preferred. Equivalent experience or education may be substituted for above requirement. Ability to plan, coordinate, implement and report all aspects of the Disease Management program; prepare concise reports; communicate effectively with members, interdepartmentally, and contract providers.
