Nursing Assistants (Hospice Care Center)

SUMMARY

The State Tested Nursing Assistant is an assistive personnel specially trained to provide personal care, meal preparation, and assistance with therapeutic routine in order to promote maximum level of independence for patient in his/her place of residence. The Home Health Aide is committed to the philosophy, goals, and objectives of the hospice concept of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Gives or assists with personal care of hospice patient, including but not limited to bathing, dressing, oral hygiene, hair care, and feeding.

Maintains or assists with maintaining cleanliness of patient's immediate surroundings.

Assists patient with prescribed exercises which patient and aide have been taught and which are supervised by appropriate professional personnel.

Recognizes and reports changes in mental or physical status of patient promptly to RN Case Manager.

Participates in patient care conferences as requested by the RN Case Manager

Assists RN Case Manager with specific assigned procedures.

Requests assistance if unable to perform assigned tasks competently.

Documents activities carried out and observations made.

Attends scheduled in-service and training sessions per regulatory requirements.

Assists in Inpatient Unit and supply room when requested.

Provides extended care or respite care as dictated by Manager, Home Care Team.

Incorporates the customer service values and standards into the daily activities and experiences with patients, families, physicians, facility staff members and co-workers.

Helps patient achieve good quality of life through encouraging independence, encouraging to eat, encouraging social contact, providing companionship, and providing reassurance.

Assists patients in immobility and transferring from bed to chair.

Participates in Continuous Quality Improvement activities (if applicable)

Precepts new staff.

Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations. Complies with all HOD policies and procedures.

EDUCATION and/or EXPERIENCE

High School Diploma or GED

State Tested Nursing Assistant Certificate

LANGUAGE SKILLS

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.

CERTIFICATES, LICENSES, REGISTRATIONS

Completion of written Home Health Aide test.

Completion of a formal Home Health Aide training program and STNA required

Valid Driver's License

Automobile Insurance

OTHER QUALIFICATIONS

Personal/Care giving Skills (Baths, Making Beds)

Body Mechanics (Transfers/lifting patients/assisting with ambulation)

Documentation Skills

Emotional maturity

Interest in and empathy for terminally ill patients
