Education & Experience Requirements: Current Nurse Practitioner license for the State of New Jersey. Three to five years of experience in a health care setting. Primary Functions: Daily evaluation and assessment of heart failure patients. Chart review and interaction with physicians to assure compliance with core measures. Working with the staff and management team and development of a comprehensive heart failure program
