CARE MANAGER RN, Region 2 Area on Aging.
Care Manager RN
Duties include: Assess needs of elderly and disabled individuals in their homes; develop plans of care, conduct ongoing monitoring, and quarterly reassessments in Hillsdale, Jackson and Lenawee Counties. Must possess Registered Nurse license from the State of Michigan and have at least one year of experience in the field of aging. Must have reliable transportation. FT. Work schedule M-F excluding observed holidays (no on-call). Union position. $21.05/hour. Full benefits (to include medical/ dental /vision insurance for employee/immediate family members).
