Responsibilities:

Responsible to provide Telephonic Disease Management - Coaching members on disease specific medical/nutricial requriements, signs/symptoms, and avoidance of complications.

Be able to assess member's readiness to change, utilizing Prochaska Model, apply appropriate intervention(s).

Establishment and follow-up of goals.

Distribution of educational materials to participants.

Data Entry of Information into designated programs.

Identification and referral of appropriate memebers to case management.

Protects the confidentiality, integrity and availability of information and records that TML IEBP creates, receives, maintains or transmits by complying with TML IEBP policies and procedures, including policies and procedures on physical, administrative and technical safeguards.
Qualifications/Skills required:
- 3 years recent experience practicing as a Certified Diabetes Educator.
- 3 years current Dietitian experience with Diabetes and cardiovascular population.
- Disease Management and/or Behavioral Healthy experience a plus.
- Knowledge of available disease specific resources.
- Advanced knowledge of endocrine and cardiovascular systems.
- CCM (McKesson Software) experience and one year Microsoft Office experience.
Salary is contingent on related experience
