World renowned health care organization located in Manhattan , NY - Committed to the highest quality of care! They are looking for a Senior Care Manager and ER Care Manager

SENIOR CARE MANAGER - 8-hour Days, Monday through Friday

As the Senior Care Manager, you will utilize your UM, D/C Planning and Care Management experience to a new Care Management Program and Software implementation.

• Responsibilities include coordinating daily review operations, performing reviews, educating, coaching and mentoring staff and acting as a liaison with physicians / practitioners to resolve medical necessity issues and impact length of stay.
• Emphasis will be on patient assessment and transitional levels of care.

Requirements

• Knowledge of DRGs, CMS and NY State Regulations as well as InterQual or Milliman criteria is required.
• A BSN is (Masters preferred) with at least 5 years of acute Med/Surg experience and a minimum of 3 years of Case Management experience is REQUIRED.
• Excellent interpersonal and organizational skills are necessary.

Care Manager – Emergency Department - 8 hour weekdays. Rotation to weekends is in the works

• Facilitate care coordination, patient through and discharge planning in an academic medical center environment.
• Care Managers are unit based and score PRIs.
• Medical records are online and very automated, they also have Allscripts software.

Requirements

• NY State RN, knowledge of Federal and State regulations, in addition to Milliman or Interqual criteria, BSN, 5 years recent med/surg (would love 5 years) and UM, Case Management (would love 3+ years) or related managed care experience.
• PRI cert a plus.
• Excellent communication and computer skills expected.
