Job Description

Position Profile
Therapist provides patient evaluation, assessment, treatment, and education according to therapist licensure, federal/state regulations, accreditation standards, and business unit policies and procedures. The therapist works cooperatively with members of the interdisciplinary team and actively participates in the coordination of services. The therapist has case by case responsibility to supervise home health aides and physical therapy assistants. The therapist participates in case conferences, team meetings, and other agency/systemwide meetings as requested. Performs other assigned duties.

Current CA PT License.


Minimum of one year recent acute care experience required. Fluent knowledge of spoke and written English required. BLS from American Heart Association.


Home Health experience preferred. Knowledge of Medicare, Medi-Cal, and other third party payor guidelines preferred. Ability to perform all baseline clinical therapy interventions and coordinate all aspects of patient care. Effective interpersonal, communication, organizational, and time management skills. Must be able to demonstrate position-required behaviors in fulfillment of the organization's mission.
