- The Patient Centered Medical Home (PCMH) Consultant works directly with designated physician practice/s as part of the APIPA/practice medical home partnership to ensure successful implementation of a medical home model which provides individualized, patient-centered, comprehensive care for APIPA members.
- Collaborating with the practice medical home clinicians, the PCMH Consultant assists the practice in identifying patients at risk for adverse outcomes and facilitates communications to ensure practice interventions are timely and effective.
- The Consultant assists the practice in developing work flows to optimize care delivery, and recommends best practices and process improvements.
- The PCMH Consultant is responsible for supporting the practice in development and effective use of a patient registry to increase its ability to track delivery of evidence based care, and monitor results across the practice population.
- The PCMH Consultant will also support process improvements to assist the practice in becoming an NCQA certified Medical Home, if desired.

- Minimum 5 years Medicaid managed care experience in state of Arizona working in an innovative primary care practice or FQHC
- Ability to analyze and monitor results of outcome measures and assist physician practice in identifying opportunities to improve the efficiency and effectiveness of care delivery
- Minimum of 3 to 5 years of recent clinical experience.
- Advanced level of computer literacy including experience with electronic medical records
- Ability to navigate without assistance in multiple computer applications and systems
- Excellent problem solving sills
- Strong interpersonal skills that enable relationship building
- Effective oral and written communication skills
- Strong persuasion skills and demonstrated evidence of innovative thinking and program development to make the health care system work better.
- Ability to work independently with minimal supervisory oversight.
- Masters degree MPH, MBA, medical informatics or similar
Desired
- Previous experience implementing a Medical Home model in a primary care practice.
- Clinician with a current, unrestricted license in the state of Arizona.
- Knowledge and/or experience of NCQA standards
