LVN
Qualifications: Current CA LVN license and BLS certification. Ability to give and follow verbal and written instructions. Ability to work effectively with the public. Demonstrated knowledge of sterile technique. Minimum one (1) year home health or hospice (Medicare Certified) or three years (3) LVN experience in the last five (5) years. Medical Surgical skills. Demonstrated ability to work independently and collaborate effectively on a multi-disciplinary team; excellent communications, problem solving & service quality skills. Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to customers, contracted providers, and vendors.
Preferred Qualifications: Bilingual (English/Spanish). Hospice and/or Home Health experience. Knowledge of family dynamics, Death and Dying issues, DME, Kaiser Permanente Info Systems. Wound care skills.
Notes: This position will include some after hours from 5pm-8:30am and some weekends. Driving 40-120 miles daily in all parts of SD County (mileage reimbursed). The LVN will spend time primarily making visits to patients in the home. A typical day may be spent as 1/2 direct patient care, 1/4 driving, and 1/4 paperwork. Valid CDL, auto insurance, and functioning auto at all times.
