DEFINITION:

The Assessment Supevisor is responsible for the administrative and clinical supervision of Assessment Workers who are responsible for the financial and clinical assessment, level of care determination, care planning and processing of consumers applying for Long Term Care services. The ideal candidate must have at least three years supervisory experience with well developed interpersonal and communication skills and a high level of professional ethics.


REQUIREMENTS:

  • Master’s degree in Social Work or related field.
  • Must have a minimum of three years supervisory experience in a social work setting, preferably in community‑ based services to individuals or hospital/nursing home social work.
  • Previous experience serving the elderly or disabled populations is preferred.
  • Must have a valid driver’s license and a good driving record.
  • Strong analytical skills with the ability to review a high volume of consumer files within mandated timeframes.

RESPONSIBILITIES:

  • Review and approve assessments, level of care determinations and care plans completed by Assessment Workers and make recommendations concerning their suitability in addressing an individual client’s long term care need.
  • In conjunction with the Nurse Consultant, supervise the Assessment Workers and the Clerk Typist assigned to your team through bi-weekly supervision, team meetings, case reviews, field observation and day-to-day contact.
  • Team with the Nurse Consultant to resolve problems and to complete performance appraisals for your team members.
  • Report and make recommendations regarding problem solving and disciplinary action to Assistant Director.
  • Work collaboratively with other agencies and individuals in assuring service delivery to consumer population.
  • Prepare written testimony and participate in appeal hearings.
