STATEMENT OF WORK – Family Nurse Practitioner (FNP)

Deployment Health Clinic (DHC) at NBHC Gulfport, MS with the focus on Post Deployment Health Re-Assessment (PDHRA) tasks, Pre Deployment Screens and Preventive Health Assessments

Qualifications:

a. Family Nurse Practitioner (FNP).

1. Be a graduate of a Master’s of Nursing program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).

2. Possess a current unrestricted license to practice as a registered nurse in one of the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.

3. Possess a current certification (in the specialty corresponding to the specialty required by the task order) as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP).

4. Have a minimum of 1 year of full-time experience within the last 3 years in a setting corresponding to the requirements of the task order.

5. Possess one year of experience within the last 3 years as a Family Nurse Practitioner in a primary care setting seeing an adult patient population.

6. Possess current certification in Advanced Cardiac Life Support (ACLS).

7. Notwithstanding the aforementioned requirements or those outlined in Section C of the basic contract, the HCW must have pertinent clinical experience within the past 2 years sufficient to demonstrate current clinical competency for the setting and procedures required by this contract.

Hours of operation.

In general, the Deployment Health Clinic operates Monday through Friday from 0600 to 1800. Services will be provided at the base in Gulfport, MS.

Staffing and scheduling Deployment Health Clinic services.

1. The Officer in Charge, or his designee, will supervise the contract HCW.

2. PA/FNP hours. The Officer in Charge, NBHC Gulfport, or his designee, will schedule all health care practitioners, active duty and contractor, to ensure adequate coverage of all clinic-operating hours. Shifts will generally be scheduled for an 8.5 to 9 hour period and include a 1/2 to 1 hour uncompensated meal break. Individual shift hours may occasionally be extended to accommodate the travel team requirements or mass screening evolution, however in no instance will the shift exceed 12 hours in length. The contractor health care practitioners shall each be on duty for about 40 hours each week, except during periods of approved leave and holidays. In no instance will the contractor health care practitioners be required to provide services (on-site service plus approved leave and holidays) in excess of 80 hours per 2-week period.

3. With the exception of the initial schedule, the specific schedule for each health care worker for each 2-week period will be scheduled 6 weeks in advance by the supervisor.

4. HCWs will each be credited for 8 hours work for each holiday (those who work on a holiday will have a paid compensatory 8-hour day off).

5. The contractor PA/FNP will accrue 8 hours of personal leave (annual plus sick) for each 80 hours worked.

6. Training. All contractor HCWs may be required to complete on-line training or attend Post Traumatic Stress Disorder or Deployment Health related classes to meet PDHRA program guidelines. Authorized training related expenses and/or travel will be reimbursed according to Section B (CLIN 0008) and C.3. 1.12 of the basic contract.

Duties.

a. Family Nurse Practitioner (FNP).

1. Provide a full range of nurse practitioner services in accordance with privileges granted by the Commanding Officer (e.g., supervise and provide general screening and medical care and examinations of patients for routine, acute and chronic conditions involving any and all organ systems; provide immunizations; diagnose, treat, and counsel patients as indicated).

2. Adhere to NAVMEDCOMINST 6550.4, Utilization Guidelines for Nurse Practitioners (available from the COR upon request). This instruction includes the following requirements: permits nurse practitioner ordering and administration of an approved list of drugs according to protocol and requires random review of records of patients seen by the nurse practitioner.

3. Treat patients with common acute conditions, illnesses, or minor trauma within accepted protocols, Nurse Practice Acts, and/or in collaboration with a physician.

4. Collaborate with the physician in the health care of patients with chronic illnesses.

5. Request consultation or referral with appropriate physicians, clinics, or other health resources as indicated.

6. Order diagnostic tests as applicable.

7. Prescribe and dispense medications as delineated by the Pharmacy and Therapeutics Committee.

8. Promote preventive and health maintenance care, including annual physicals, positive health behaviors, and self-care skills through education and counseling.

9. General. As directed, all contract HCWs shall participate in outreach and marketing of the PDHRA program and services provided. As such, the health care workers shall liaison with the Line Commanders, screen patients, provide treatment when appropriate, educate returning units on the availability of the service and coordinate follow- on care.

10. Liaison. All HCWs shall travel as directed to various branch clinics or Line Units within a 35-mile radius of the NBHC. Additionally, HCWs may be required to travel to provide temporary services at outlying clinic locations beyond 35 miles, and overnight stay may be required. Travel to the outlying locations beyond a 35 mile radius of the NBHC will be reimbursed according to section B and shall be in accordance with (IAW) with Section C.1 1 of the basic contract.

11. All HCWs shall communicate with the Line Commanders to make them aware of the Deployment Health Clinic benefit, clarify who is eligible to receive services, and to initiate the screening process. Communications will take place at the clinic and “in the field,” with the Line units.

12. The contractor HCWs shall coordinate with MTF assigned personnel to schedule initial patient screenings.

13. Screening. The DHC contractor HCW shall provide deployment health screening services to all service members either individually or in groups, at the clinic or at the Line unit as determined necessary.

14. The contractor PA/FNP shall perform Deployment Health screenings, assess the results, treat patients, and refer patients for further treatment as appropriate. An electronic or web-enabled DD Form 2900 (or Standard Form 600 if electronic or web-enabled access is not available) shall be utilized for post deployment health reassessments, an electronic or web-enabled DD Form 2795 shall be utilized for pre-deployment assessments, and an electronic or web-enabled DD Form 2796 shall be utilized for post deployment screenings.
