Job Description
JOB DESCRIPTION: Independently assesses, coordinates, and implements a timely, safe patient discharge plan to the appropriate level of care. Participates in interdiscplinary approach to patient care management. Provides emotional support and psychosocial counseling to patients. Perform other duties as required.
MINIMUM QUALIFICATIONS: Masters of Social Work degree from an accredited school of social work. One year of medical social work experience in an acute care setting. Positions in this classification require license or license eligibility in the state of Georgia (LMSW). Knowledge of computer software applications.

LMSW or LCSW with at least one year of experience working in a health care environment. Inpatient and outpatient experience with adult transplant patients highly desirable. Applicant must have experience leading clinical team in casemanagement and should possess strong clinical skills in areas of assessment, discharge planning and community resource coordination. Position requires excellent communication skills and focus on collaborative practice. Computer skills are essential. Assignment includes coverage of inpatient and outpatient lung transplant service, and discharge planning for a med/surg and critical care unit, with occasional on-call, weekend / holiday coverage.
