The Team Nurse provides skilled nursing services specific to the needs of the patient, including instruction and evaluation in the home, to a designated group of patients under physician direction. The Team Nurse is accountable to the Clinical Supervisor for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols.


Education:
RN graduate from an accredited curriculum. Bachelor’s degree in nursing or health related field preferred.
Licensure:
Current California RN license; valid California drivers license with proof of current auto insurance; current BCLS/CPR certification.
Experience:
Minimum of one year of experience in an acute care setting is required. One year of experience in a home health or hospice position preferred.
Knowledge:
Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care. Exhibits knowledge of patho-physiology and accepted treatment protocols for common home care diagnoses. Knowledge of safety and infection control principles. Knowledge of state and federal home care regulatory guidelines preferred.
Special Skills/Equipment:
Current California RN license. Valid California driver’s license and access to a reliable automobile with proof of liability insurance. Demonstrated clinical assessment skills. Ability to recognize the special needs of Hospice patients and others in the home. Ability to communicate with patients and significant others in an effective, mature, caring manner. Ability to maintain harmonious constructive relationships with internal and external customers. Demonstrated effective written and verbal communication skills. Understanding of palliative care for the terminally ill patient. Must be able to recognize unsafe or emergency situations and act appropriately. Participation in a training program for Hospice volunteers or coursework in death and dying/hospice care is preferred.
Physical Requirements:
Essential job duties and physical demands include and are not limited to: Administers skilled nursing patient care in a home or assisted living environment; following written or oral instructions; assists patients in walking and transfer from bed to chair and in patient positioning. Must be able to remain sedentary for approximately 50% of the day, maneuver around objects, climb stairs and/or work in relatively small spaces, bend over or stretch as necessary in care provision, operate telephone and manipulate small clerical items (e.g. paperclips, staples, etc.); ability to raise, lower, or transfer objects and supplies from one level to another; transport an object usually by hands, arms or shoulders; ability to bend, stoop, twist and lift and transfer up to 50 pounds in patient care. Must be able to meet company productivity standards as indicated by organization and manager. Must be able to hear, read, write and speak clearly to effectively communicate with department staff, other Sutter VNA & Hospice employees and community, and carryout essential functions. Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, think critically to both effectively plan and organize department operations consistent with Sutter VNA and Hospice’s strategic and operating objectives and to effectively solve unique problems as they arise or identify when to consult supervisor. Must be able to deal with challenging work environment with time demands and occasional conflicting priorities. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements. Is responsible for maintaining all required licensure and certifications. At orientation and annually thereafter, must be able to meet Sutter VNA & Hospice core competencies for position and area of specialty, as appropriate. Must have reliable transportation.
