Marian has 1300 employees and 250 active physicians who treat more than 100,000 patients each year. Marian's specialty services include Marian Heart Center, Marian Cancer Center, Marian Home Health & Hospice Services, and Marian Family Birth Center. Marian also provides two community clinics focused on serving the disadvantaged, a multi-disciplinary Rehabilitation Center, a Health and Wellness Center, and an Urgent Care Center. |

Case Manager plans, organizes and manages a patient caseload: applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs. Provides direct patient care through use of nursing process, performs established nursing intervention utilizing current clinical knowledge. Initiates and participates in patient, family and staff education. Demonstrates leadership capabilities. Serves as preceptor for orientees and students. Major responsibilities are listed; however, other duties may be assigned.
Qualifications: Education: California Registered Nurse license. Current CPR certification and current California driver's license with proof of liability insurance for automobile. Experience: At least one year as an RN; completion of Hospice Orientation Program; home care experience preferred.
