RN Home Care-Hospice Registered Nurse Case Manager VNA & Hospice of SVMC .06 FTE (48 hrs per pay period) M-F Days; On call share weekends & holidays Provision of skilled nursing assessment, planning, and care to home care and hospice patients and their families in order to maximize the comfort, well-being and dignity of patients and families consistent with VNA & Hospice policies and procedures. Provides supportive care and appropriate services in accordance with the attending physician's orders and interdisciplinary team plan of care. A.S. in Nursing, BSN preferred. Two years varied work experience as a professional nurse; preferably one of the two years in an acute care setting. Current license to practice as a registered professional nurse in the State of Vermont. Valid driver's license; reliable transportation. Current license/eligible to practice as a Registered Nurse in the State of Vermont . Home Health and/ Med-Surg experience preferred. Valid driver's license. EOE
