Essential Duties:
Ensures prompt admission of new patients to home health service, with development of the plan of treatment for the patient’s care, scheduling of visits, and developing organized, outcome-based teaching plan. Receives daily reports from team members regarding patient care. Coordinates services rendered to the patients by all disciplines. Reviews daily field staff documentation for accuracy, quality, and compliance with applicable standards and agency policy. Manages physician and patient phone calls with coordination of orders received with the field staff. Coordinates supervisory visits of LVNs and aides as needed and according to agency policy. Re-evaluates patient care as needed, prepares recertification, plan of care, and conducts multidisciplinary care conferences for each patient.

Participates in development of Home Health Aide care plan. Arranges RN post-hospitalization visits for patients assigned to their team. Assigns patient visits to team members according to ordered frequency and teaching plan for each patient. Coordinates with the patient/family and the team members the plans for the patient’s discharge when appropriate and completes the discharge summary at the time of DC. Assists with the performance evaluations of team members. Initiates complaint forms for all patient complaints, forwards it on to Director of Patient Care and participates in the problem resolution. Takes call for all the agency patients according to the rotating call schedule. Maintains knowledge of all applicable Federal, State, and Local laws and complaint procedure requirements. Observes all scheduled time frames for submitting accurate required paperwork in order to facilitate operations.

Complies with OHS policies and procedures. Participates in staff meetings and In-Service Education programs. Seeks opportunities to enhance professional growth. Recognizes and follow the appropriate communication and organizational chain of command.

Skill Required:
Must currently be licensed as a RN in the state of Texas. Shall have at least one year experience in an acute care setting. Shall have strong documentation and verbal skills to facilitate appropriate communication in varied situations Shall have basic understanding of Medicare rules and regulations for home care and Medicare guidelines for home health coverage and compliance.

Minimum Qualifications:
Registered Nurse currently licensed in the state of Texas. Maintains current Texas Drivers License.
