General Summary: The Anticoagulation Clinic Program Manager is responsible for program development, oversight and management of PPD Anticoagulation Services statewide. Provides leadership within the PPD to achieve high quality, safe, and cost-effective anticoagulation services. Plays an integral role in the continued improvement of anticoagulation operational efficiency, patient management software tools, and business development. Serves as a liaison throughout Providence Health & Services to improve integration and management of anticoagulation across the continuum.
Required: Graduate from an accredited College of Pharmacy. Pharm.D. or M. S. degree in pharmacy with residency required unless equivalent experience and education can be documented. Valid pharmacy license by the Oregon State Board of Pharmacy. Minimum of two (2) years experience and progressive responsibility in pharmacy management including Supervisory/Coordinator experience, demonstration of supervisory and administrative ability shown during that period.
Preferred: Minimum three (3) years experience in ambulatory or acute care pharmacy practice which included significant responsibility for anticoagulation services.Demonstrated experience and success in operational process improvement.
