Healthy Families of Florida Family Support Worker: Childhood Development Services

This position is responsible for initiating and maintaining regular long-term home visiting services and links to other needed support services for the physical well-being of children who are at risk of being abused/neglected, and their families.

ESSENTIAL JOB FUNCTIONS:

1. Establish trusting relationships with at-risk families by initiating and maintaining regular contact with families.
2. Maintain full caseload ranging from 15 to 25 families.
3. Maintain all charts in a safe and confidential manner as prescribed by the program.
4. Attend regularly scheduled weekly supervision meeting with the supervisor.
5. Conduct home visits as prescribed by the Healthy Families Florida leveling system.
6. Promote positive parent-child interaction through parent education and modeling appropriate behavior during each home visit.
7. Provide in-home parent education to participating families using the Growing Great Kids (GGK) curriculum and supplementing with appropriate curricula approved by Healthy Families Florida.
8. Make appropriate referrals to families and follow-up with families regarding the effectiveness of referrals made. Act as a liaison between families and other community agencies.
9. Administer the required tools within the designated intervals as required by Healthy Families Florida.
10. Document home visit observations and activities.
11. Perform all necessary record keeping and reporting functions as required by Childhood Development Services policies, Healthy Families Florida policies and Florida Statute Chapter 39.
12. Ensure all paperwork is completed in timely manner.
13. Complete required program related trainings.
14. Minimal travel may be required.

MINIMUM QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

1. High School Diploma or equivalent
2. One year experience working with diverse children and families providing services.
3. Experience establishing trusting relationships and accepting individual differences while working in the community.
4. Knowledge of infant and child development.
