MDS PPS Coordinator
RN


We are seeking a highly motivated and dedicated nurse to join our team. Would prefer someone with experience in reimbursement, great organizational skills, detail oriented. The PPS Coordinator works with the interdisciplinary team to coordinate a comprehensive assessment for each resident, and ensures that all assessment triggers are addressed. Also, the PPS Coordinator makes certain that all skilled care needs are identified and met with services.

Responsibilities include:

* Complete assigned MDS items, RAPS and Care Plans
* Direct the facility interdisciplinary RAI process while following guidelines for completion, computer encoding and electronic State transmission of the MDS.
* Participate in Resident Care Plan Meetings.

Requirements include:

* Must be a Registered Nurse in good standing and currently licensed by the State of Massachusetts
* Previous experience in long-term care
* Must be knowledgeable of Federal requirements for long-term care, rehabilitative and restorative nursing and medical practices and procedures, regulations and guidelines governing long-term care
* Possess strong knowledge of the Managed Care Process
* Previous experience as an MDS Coordinator is preferred
