We're seeking a flexible Case Manager to join our Moline team and aid us in changing our patients' behavior regarding personal care. As a Case Manager, you would make outbound calls to high risk members with chronic diseases. This position is ideal for an RN with acute care experience and who enjoys daily research and telephonic client contact. If you are looking to make a difference in someone's life, this could be the position for you.
Key responsibilities include:
- Overseeing a daily case load
- Developing and focusing on gaps in patient care
- Educating patients on personal care
- Assisting in home health set-up
- Working with physicians on issues that the patients may be having
- Aiding the patients in solving issues such as transportation, social problems, etc.
- Sending welcome packets to patients
- Working with the patients' family and physician to coordinate care
Qualifications The chosen candidate must possess:
- A current, unrestricted RN license in Illinois
- 3+ Clinical experience in an acute care, home care, med surg or long term care setting
- A solid ability to prioritize and multitask
- Proficiency in MS Office applications
- Internet researching skills
- Excellent communication skills
- An ability to diffuse escalated situations
- Strong critical thinking skills
Preferred assets include:
- BSN
- Bilingual Spanish
- Certified Case Manager (CCM)
- Prior case management experience
- A background working in a managed care role or setting
